Use this Consumer Library Information Prescription (CLIP), a free medical information service provided by Saint John’s Health System to all patients, their families and/or caregivers. This service combines the consumer health & medical collections of the Saint John’s Health Sciences Library with the wealth of materials available online through the National Library of Medicine.  When you fill in the form below about the specific disease, surgery, test or research topic that you have questions about, our Librarian will retrieve accurate, relevant, up-to-date websites or material for you.  This information will be easily understandable, and can be emailed, faxed or mailed to you (or brought to the hospital room of an SJHS inpatient).

To get information, please fill out the form below, then click on "Submit".  This form is secure, and the personal information will only be used to answer your question.  You must fill in at least the topic or question you'd like information on, and one way to contact you (email address, fax number, mailing address or hospital room number).  The other questions are optional, but will help us find the best information for you.
