DESCRIPTION:
As a
public service, Saint John’s provides forms which
enable parents to make medical arrangements for their
children if an emergency arises while the parents are
away or a child is traveling without them.
The
consent and medical history form provides the necessary
information and authorization for prompt treatment. The
form should be notarized and kept with the child or
guardian. Forms are available by calling 649-2511 or
646-8290, and in literature racks throughout the Medical
Center.
